PATIENT CARE MANAGEMENT

Each patient has a dedicated clinical and operational team who is responsible for assuring the quality and cost-effectiveness of their care needs. The team is led by the Director of Patient Services who is responsible for coordinating all the patient’s services from admission to discharge. They communicate on a regular basis with the patient, physician, pharmacist, and other members of the healthcare team. The Director of Patient Services also contacts the insurance case managers to keep them apprised of the patient’s clinical condition and ongoing care needs and to obtain the necessary authorizations for service.

COMPREHENSIVE PATIENT EDUCATION AND TRAINING

Ameritech Homecare Solutions LLC strives to foster independence in the patient and caregiver in administering prescribed therapies and using specialized equipment. Patient education begins at the time that the referral is received. The course of training and education is based on the patient’s individual needs. Each patient receives a customized training packet with therapy-specific educational resources. The pharmacist and nurse work collaboratively to provide the patient with comprehensive information regarding medications, home health care, supplies, and equipment. The nurse monitors compliance on each subsequent visit and reinforces the education process.

CRITERIA FOR ADMISSION TO HOME HEALTH CARE

  • The patient must be under the supervision of a physician who will participate in the care plan on a continuous basis.
  • The patient must have a need for intermittent skilled service.
  • The patient must be a resident of the counties we serve.
  • Based upon a home health care assessment, it is determined that the patient can be cared for safely at home.
